These forms are REQUIRED by schools participating in the Wakakirri National Story Festival.
FORMS
Story-Dance forms
• 2008 Publicity & Consent Form (every participating student, teacher and helper is required to complete this form)
• 2008 APRA Music Statement - Primary schools (Due Friday 2 May)
**Please ensure that in the 'Item Duration' section of your APRA FORM you include the duration of music WITHIN your item, NOT the duration of the entire song you are using.

• 2008 Story-Dance Principal's confirmation of entry (Due Friday 2 May)
• 2008 Story-Dance Judge Information Sheet (You must bring this form to your rehearsal)
• 2008 Lighting Design Sheet (You must bring this form to your rehearsal)
• 2008 Set & Prop Query Sheet coming soon (Optional form)
Throughout the production process you will also be sent these other forms specific to your school:
